America General - Travelling after 3 months of stay in US

Hi,
I need to travel to my home country after 3 months on medical grounds.
I want to know do I need to do some paperwork before leaving US.

I want to know what the rule of reentry.
Do I need to reenter in 1 year period of leaving the US or
Do i need to stay for 6 months in 1 years period to maintain my status.

I just want to know my options to make better desicion ?

Thanks Raveen/Vietnam Kid.

Just to clarify I want to maintain my "Permanent Resident" or "Greencard" Status.

I have heard that we cannot file for reentry permit while outside US.
I have to to go home in next 3 weeks and I think it will take 120 days to get same
Please let me know how should I proceed to get reentry permit.
Anyone with similar experience will be of lot help.

Dear Delhi Walla, if you are going to stay out of US for less than 1 year, then you do not need Reentry Permit. The issuance of permit itself takes much longer time and it is expensive. see the following site:
